Services:
Hospice Care: End-of-life care for those with an expected life expectancy of 6 months or less if the disease runs its normal course.
Palliative Care: Care for those that may have longer than 6 months to live but need assistance managing symptoms and problems associated with a serious chronic illness.
Home Health Care: Skilled nursing care, physical therapy, occupational therapy, speech therapy, medical social work, aide services all need to help restore a person to health or maintain their current level of functioning. These services require a physician's order.
Daily Living Services: Homemaking or housekeeping services that are covered by Medicaid or privately paid.
PASSPORT Provider
NO ONE WILL BE TURNED AWAY BECAUSE OF THEIR INABILITY TO PAY FOR SERVICES. We work with a majority of insurance companies to provide care.
REFERRALS: Referrals may be made by physicians, community agencies, family or prospective patients.
